Social prescribing

Our Social Prescribing Service helps patients to get the right support. Many patients could benefit from a referral to Social Prescribing.

They include:
- People with mild or long term mental health problems
- Vulnerable groups
- Those who feel lonely or isolated
- Those who frequently attend Primary or Secondary care
- Those in a caring role

Once you have been referred into our Social Prescribing Service they will contact you within 10 working days to discuss the referral and arrange an appointment if necessary – this will usually be at your home or a neutral environment.

They will help identify areas in your life where local activities, practical and emotional support could help you to feel better, more confident and more able to manage your current situation.
